At the 2510 North 9th Street St Louis, MO construction site, an employee working on a ladder jack scaffold thirty (30) feet above the ground did not have a separate ladder in place to access the scaffold. The employee climbed up one of the ladders supporting the ladder jack scaffold and then climbed over the pick board to access the platform.
