29 CFR 1910.147(c)(7)(iv): The employer did not certify that employee training had been accomplished and kept up to date:  (a) Production Floor - On or about June 19, 2013, authorized level training for employees who perform servicing and maintenance on equipment with sources of hazardous energy, to include the Mori Seiki Mills and Lathes, was not certified to demonstrate completion.  (b) Production Floor - On or about June 19, 2013, affected training, for employees who operate equipment which are subject to servicing and maintenance and which have sources of hazardous energy, was not certified to demonstrate completion.
