Two employees performing roofing work were standing on the scaffold between adjacent roof brackets when the scaffold collapsed.  Observed on or about 6/21/2013.

The maximum intended load of one worker between every two adjacent support brackets was exceeded when two employees were working between two adjacent support brackets.
