29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed  according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide.  a) Located along the side exterior area of the structure employees were observed working from a self-supported tubular welded frame scaffolding system with a 38" wide minimum base erected to aheight in excess of 19' without the scaffolding system being secured to the structure or equivalent.

29 CFR 1926.451(e)(6)(i): Integral prefabricated scaffold access frames were not specifically designed or constructed for use as ladder rungs:  a) Employees were observed gaining access to scaffold work platforms by climbing walk-through end frames which were not specifically designed and constructed for such use in that rung lengths were less that 7", rungs were not uniformly spaced and rung spacing exceeded 16 3/4" exposing employees climbing the end frame legs to fall potentials in excess of 13'.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) Located along the side exterior area of the structure a self-supported tubular welded frame scaffolding system was not erected and used properly under the supervision of a competent person in that protection from tipping and proper safe access was not erected or utilized.
