29 CFR 1926.501(b)(13):   29 CFR Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented.      Job site located at 7724 S. 162nd Ave, Omaha, NE:  Five employees and the owner were observed conducting roofing operations on a residential structure without fall protection.  They were exposed to falls of 10 feet or more.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(1):   Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder�s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.  Job site located 7724 S. 162nd Ave, Omaha, NE.:  An extension ladder used for access and egress to roof did not extend at least 3 feet above the surface being accessed.  The ladder was also not secured at its top to a rigid support.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(4):   Ladder(s) were used for purposes other than the purpose for which they were designed:  Jobsite located at 7724 S. 162nd Ave, Omaha, NE:  The top section of an extension ladder was being utilized to access a roof surface.  The ladder is not designed to be used in separate sections as outlined n the labeling on the ladder side rail.     Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.
