Listed or labeled equipment were not installed and/or used in accordance with any instructions included in the listing or labeling:    (a) 132 Walnut St Ext Agawam, MA: On or about July 17, 2013, a grounded to non grounded electrical adapter was used to supply power to a microwave and an additional household rated multi outlet extension cord neither of which are in agreement with their labeling/listing and are not in accordance with this subpart.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a) 132 Walnut St Ext Agawam, MA: On or about July 17, 2013, there were several instances discovered where various extension cords were used in lieu of permanant wiring to supply electrical power about the establishment to microwaves, mixers, refrigerators, etc., which is not in accordance with this subpart.
