29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    On or about June 26, 2013, at 601 Fairview & Curzon, in Fort Worth, Texas employees installing a new 8 inch sewer line  inside a trench were not protected from falling rock/dirt when the spoil pile and other materials were located along the south side of the trench.  Employees were exposed to struck by and cave-in hazards.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:      On or about June 26, 2013, the employer did not ensure that the ladder inside the trench box extended 3 feet above the upper landing surface.  Employees were exposed to fall hazards.
