29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:       (a)   Production Services International @ 12th St & Acoma St in Denver, CO 80012: On and before 6/24/13, the employer did not ensure that employees were equipped with personal fall protection. Employees are exposed to a fall hazard of approximately 23 feet while untying the latch from the banner support and the basket of the forklift.       Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
