29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer shall ensure that each operator has successfully completed the training required by this paragraph (l), except as permitted by paragraph (l)(5).      a) Powered industrial vehicle training was not provided prior to allowing employees to operate equipment including but not limited to  forklifts, cherry pickers and electrical powered pallet jacks.    b) Powered industrial vehicle training was inadequate in that the training did not consist of the elements of 1910.178(l)(2)(ii) in that the training was not formal instruction (video tape or writen instructions) with a written test to ensure that knowlege was obtained.
