29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level.    On or about June 27, 2013 at 11308 SW 5th St., Yukon, Oklahoma worksite, employees working at approximately 15 ft. above the ground on a fabricated welded frame sectional scaffold performing bricklaying operations were not protected by a complete guardrail system or equivalent means of fall protection.  Employees were exposed to fall hazards.

29 CFR 1926.453(b)(2)(v): A body belt was not worn worn and a lanyard attached to the boom or basket when working from an aerial lift.  On or about June 27, 2013 at 11308 SW 5th St., Yukon, Salamanca Solis Masonry did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the Genie S-65 telescopic boom aerial lift exposing employees to a fall hazard of approximately 15 ft. to the ground below.
