On or about June 27, 2013 at 11308 SW 5th St., Yukon, Salamanca Solis Masonry did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the Genie S-65 telescopic boom aerial lift exposing employees to a fall hazard of approximately 15 ft. to the ground below.
