29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a) 308 Route 5, Chittenango, NY - On or about 1 July, 2013 - Employees were using a ladder that did not have the side rails extended a minimum of 3 feet above the landing surface the side rails only extended approximately 12 inches above the landing.
