29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Location: 1009 Flatbush Avenue, Brooklyn, NY. Bottle Room. The employer did not implement Lock out /Tag out procedures to isolate the CF 1500 plastic bottle machine from its power source. The Lock out /Tag out procedure was deficient in that it did not address the unjamming of the bottle machine. On or about 07/01/2013.

Bottle Room. Employees did not receive Lockout/Tag out training while maintaining and servicing the Emvipco CF 1500 plastic bottle machines. The employer had not trained the employees to ensure the purpose and function of energy control program was understood. On or about 07/01/2013.
