29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  The employer authorized production employees to operate the powered industrial trucks failing to provide training that included a practical training, (demonstration performed by the trainer and practical exercises performed by the trainee) and an evaluation of the operators performance in the workplace.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
