29 CFR 1926.100(a):       Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:       a)    At the worksite at approximately 1004 Fifth Street, Alton IL, an employee, was exposed to head injuries and was not wearing a protective helmet.  The employee was beneath other employees that were elevated in an aerial lift.  They were installing sections of metal and were using hand tools and portable drills that could drop onto the employee.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:     a)    At the worksite at approximately 1004 Fifth Street, Alton IL, an employee  was using a 6 foot step ladder by leaning it against the building and then climbing the steps to reach higher elevation.  The ladder was not folded to be used as it was designed.
