29 CFR 1926.502(f)(1)(iv): When the path to the point of access was not in use, a rope, wire, chain, or other barricade, equivalent in strength and height to the warning line, was not placed across the path at the point where the path intersected the warning line erected around the work area, nor was the path offset such that an employee could not walk directly into the work area:    On or about 07/03/2013 at the Salem School Construction Project, there was no rope or other barricade placed across the path to the ladder in use to access and descend to and from the roof.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about 07/03/2013 at the Salem School Construction Project, employees were accessing the upper roof from the lower roof and were stepping up 23 inches from lower roof to upper roof.
