29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   This violation occurred July 5, 2013 at a jobsite located at 834 West Centerville Road, Garland, TX  where employees performing cornice repair work were not protected from falling more than 17 feet to the lower level.

29 CFR 1926.1053(b)(1): Where  portable ladder was used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:  This violation occurred on July 5, 2013 at a jobsite located at 834 West Centerville Road Garland, Texas where employees using a portable extension ladder were exposed to a fall hazard.
