Section 5(a)(1) of the Occupational Safety and Health Act of 1970. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employees were exposed to fall hazards while working on and from a job-made platform with no fall protection systems in place.   a) On or about 05/31/13 two employee were observed working on and from a job-made work platform which was inadequately secured, resting on the forks of a GEHL Dynalift Rough Terrain Forklift truck, elevated in excess of 20 feet above the ground below with no fall protection system in place.
