29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:       (a) Jobsite: 2617- 2621 Josephine Street, Pittsburgh, PA 15203: On or before July 2, 2013, employees conducting roofing activities were exposed electrical shock and arcing part hazards in that a 2460 volt AC field phase conductor was located within approximately 6 feet 2 inches of the structure at roof level.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a) Jobsite: 2617- 2621 Josephine Street, Pittsburgh, PA 15203: On or before July 2, 2013, an employee removing roofing materials was working unprotected and exposed to fall hazards of approximately 20 feet 7 inches while on a 6/12 pitched roof.

29 CFR 1926.503(c): The employer did not retrain affected employees  who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section:     (a) Jobsite: 2617- 2621 Josephine Street, Pittsburgh, PA 15203: On or before July 2, 2013, the employer did not re-train their employee on the hazards associated with falling while removing roofing materials on a steep sloped roof and what fall protection methods shall be used for the entire job duration.
