29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a) Plant- Throughout the plant: Employees are exposed to caught-in and shock hazards while servicing machinery such as Tufting Machines, Yarn Machines as well as change-out of electrical panels without a hazardous energy control program.

29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a) Plant - Backing Department: Employees were exposed to crush hazards while free rigging using two 2 inch wide rubber drive belts in conjunction with forklifts to hoist backing on backing bar into place during change out. The backing weighs 800 lbs and the bars are between 35 to 70 lbs. The belts are not rated for lifting loads and no manufacturers approval has been provided to perform free rigging.

29 CFR 1910.212(a)(5): Exposure of blades. When the periphery of the blades of a fan is less than seven (7) feet above the floor or working level, the blades shall be guarded. The guard shall have openings no larger than one-half (1/2) inch. Employees were exposed to rotating fan blades due to the rear baffles of the floor fans being removed.
