29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets:   a)  Rockville Water Treatment Plant - On or about 7-3-13 - Employees were observed working on the flat roof of a pump house, 21 feet by 19 feet, exposed to a 13 foot fall hazard with no fall protection system being used.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a)  Rockville Water Treatment Plant - On or about 7-3-13 - Employees were observed working on pump house roof using a 24 foot aluminum extension ladder to gain access to the roof that did not extend three feet above the landing surface.
