29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) In the facility's warehouse area, on or about July 08, 2013 - Employees engaged in machining piecework on a LeBlond Regal Lathe (No ID- Machine 110) were exposed to struck-by hazards when operating the machine without guarding.    b) In the facility's warehouse area, on o about July 08, 2013 - Employees engaged in machining piecework on a Mazak 24-100 Lathe (NO ID- Machine 4141) were exposed to struck-by hazards when operating the machine without guarding.    c) In the facility's machine shop, on or about July 08, 2013 - Employees engaged in polishing work, bypassing primary guards on a Mazak CNC Machine (MN QTN450-II SN 236502), were exposed to struck-by and caught-in hazards due to rotating parts, which were in operation while unguarded.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven feet or less above floors or platforms were not enclosed:    a) In the facility's machine shop, on or about July 08, 2013 - Employees engaged in machining work on the Mazak Conveyor System (MN QTN3502/KOD/1500 SN 232902) were exposed to caught-in hazards when the chain and sprocket assembly was not guarded.
