29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:      a) On or about 7/09/13, in the "Bullpen Area" on the fourth floor of the Larkin Center of Commerce Building, 701 Seneca St.,Buffalo, NY: A Dayton 30 in. Circulator Fan, Model 6C539 was used to provide ventilation. The Fan was connected to 120-volt ac power via a series of extension cords. The plug on the cord of this Fan lacked a grounding prong. Seneca Larkin 701 LLC owned the fan and provided it for ventilation. Employees of other contractors walk by this fan daily.         b) On or about 7/09/13, in the "Bathroom Area" on the fourth floor of the Larkin Center of Commerce Building, 701 Seneca St.,Buffalo, NY: A Leading Edge Fan, Model LPM3025 was used to provide ventilation. The Fan was connected to 120-volt ac power via a series of extension cords. The last extension cord in the series was a coiled yellow "2-wire cord" that did not contain a grounding conductor.  This fan also contained two power cords with attached plugs, one of which was connected to 120 volt-ac power. The other plug was missing the ground prong & the neutral prong, but was not connected to power.  Employees of other contractors walk by this fan daily.      NO ABATEMENT CERTIFICATION REQUIRED
