29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    a. Where saws and other equipment, such as conveyors, were repaired, adjusted, and other maintenance performed, a lockout program was not developed and implemented, including ensuring locks were applied by everyone working on the equipment, documenting training, annually inspecting the program (and documenting), and putting the program in writing.    Abatement documentation is required for this violation.

29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  a. Where saws were operated, a noise monitoring program had not been implemented to determine the need for annual hearing tests.  (Note: to abate this violation annual hearing tests can be provided without conducting a noise monitoring program.)  Abatement documentation is required for this violation.
