29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation:    a) On or about 07/10/13 CSHO observed standing water in a trench/excavation with no means in place to dewater. Employee who was working in trench/excavation installing saddle on sewer line and was observed and told to get out of trench by Woodbridge Senior Public Works Inspector prior to CSHO arriving on site. Second employee stated to CSHO during interview he was also in the trench/excavation.

Santos Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(k)(1), which was contained in OSHA inspection number 313985608, citation number 1, item number 1 and was affirmed as a final order on 01/17/2012, with respect to a workplace located at 511 Lyman Avenue, Woodbridge, NJ 07095.

Excavation was not made entirely in stable rock and exceeded 5 foot in depth:        a) On or about 07/10/13 CSHO observed a trench/excavation, 2 foot, 5 inches wide by 7 foot, 5 inches deep and approximately 35 foot in length with no protective system in place. Employee was told to get out of trench/excavation by Woodbridge Senior Public Works Inspector prior to CSHO arriving on site. Second employee stated to CSHO during interview he was also in the trench/excavation.          Santos Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 313985608, citation number 1, item number 2 and was affirmed as a final order on 01/17/2012, with respect to a workplace located at 511 Lyman Avenue, Woodbridge, NJ 07095.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identified them as defective, or tagged with Do Not Use or similar language, and were not withdrawn from service until repaired:       a) On or about 07/10/13 CSHO observed an aluminum ladder (fly section) being used for access and egress out of trench/excavation when the ladder was damaged and not removed from jobsite and or tagged Do Not Use.  Employee was told to get out of trench/excavation by Woodbridge Senior Public Works Inspector prior to CSHO arriving on site. Second employee stated to CSHO during interview he was also in the trench/excavation and used ladder for access and egress.           Santos Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(16), which was contained in OSHA inspection number 313985608, citation number 1, item number 4 and was affirmed as a final order on 01/17/2012, with respect to a workplace located at 511 Lyman Avenue, Woodbridge, NJ 07095.
