29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    a)  1091 Todd Plaza, Lakewood, NJ:  Employee acting as a helper to the employee in the truck mounted articulating boom was not wearing a hard hat when exposed to overhead hazards.  The employee was handing long light bulbs to the employee in the basket, observed on or about 7/9/13.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift:     a)  1091 Route 9, Lakewood, NJ:  Employee performing sign repair was working from a truck mounted articulating boom lift was not wearing a safety harness attached to the boom lift.  The employee was working at a height of greater then 10 feet from the ground level, observed on or about 7/9/13.       Stand-Out Signs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.67(c)(2)(v), which was contained in OSHA inspection number 311914196, citation number 1, item number 1 and was affirmed as a final order on 10/16/2009.
