29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift; Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:   a)      Employees who were working on second story roof surfaces were accessing the roof surface using a JLG Tow Pro aerial lift without fall protection equipment when in the basket.    ABATEMENT CERTIFICATION REQUIRED.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)      Employees were working on a second story 7:12 pitch roof with an eave edge twenty (20) feet above compacted surface without utilizing fall protection equipment.    b)      Employees who were accessing a second story roof were walking on an attached second story 7:12 pitch roof with an eave edged twenty (20) feet above compacted surface without utilizing fall protection equipment.      ABATEMENT CERTIFICATION REQUIRED.
