29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour.    On or about July 11, 2013, in the Fabrication shop, the employer allowed employees to store oxygen and acetylene gas cylinders together on a cart with the regulator gage on them.

29 CFR 1910.253(e)(1): Equipment was not installed and used only in the service for which it was approved and as recommended by the manufacturer:  On or about July 11, 2013, at Wilco Fabrication, the employer allowed employees to use pressure gages on gas cylinders that were missing the protective covering.   1) In the Pre-Fab area the pressure gage for the gas cylinder was missing the protective covering  2) In the Fabrication area the pressure gage for the acetylene gas cylinder was missing two protective coverings.
