Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a) Colonial Square Apartments, Building H; on or about 7/11/2013:  Employees were installing roofing material on a flat roof without use of conventional fall protection and/or a safety monitoring system; employees were exposed to fall hazards of up to 22 feet.
