29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures    a) 844 N Broad St, Sidewalk on left facing road - Employees replacing a house trap in an excavation with undermining of the sidewalk of approximately two (2) feet were exposed to struck-by hazards as an adequate support system or method was not utilized.  On or about July 10, 2013.    b) 844 N Broad St, Sidewalk on right facing road - Employees replacing a house trap in an excavation with undermining of the sidewalk of approximately one (1) foot were exposed to struck-by hazards as an adequate support system or method was not utilized.  On or about July 10, 2013.    c) 844 N Broad St, Under road - Employees replacing a house trap in an excavation with undermining of the road of approximately seven and a half (7 1/2) feet were exposed to struck-by hazards as an adequate support system or method was not utilized.  On or about July 10, 2013.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions.    a) 844 N Broad St, Sidewalk - Employees replacing a house trap were exposed to struck-by and cave-in hazards as daily inspection of excavation and protective systems were not made by a competent person.   On or about July 10, 2013.
