29 CFR 1910.23(d)(1)(iv): Stairway(s) more than 44 inches wide, but less than 88 inches wide having both sides open, did not have one stair railing on each side:      a) On or about July 11, 2013 and at times prior, employees using the stairways, approximately 48 inches wide,  on the south side of the Front building were exposed to unguarded open sides in that the flights of stairs did not have the required stair railing or handrails in the following locations:       Set #1 The 5 riser high concrete stairs that lead out of the polish area to the outside did not have any stair rails on its open sides.       Set #2 The 6 riser high concrete stairs used to travel from the west bay area to the outside and then into the doorway of  the Main shop area had only one hand rail on its south side.        Abatement Note:   Abatement certification is required for this item. (See enclosed Certificate of Corrective Action worksheet).

29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300) and for the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment:   a) Perkins Paint & Collision did not maintain a Log of Work-related Injuries and Illnesses (OSHA from 300) and/or the Summary of Work-Related Injuries and Illness, (OSHA form 300-A) or equivalent for the establishment for the year of 2012. On 8/21/2012, a body technician had a day way from work due to a foreign object in the eye.     Abatement Note:   Abatement certification and documentation are required for this item. 29 CFR 1910.1200(f)(1): The chemical manufacturer, importer, or distributor did not ensure that each container of hazardous chemicals leaving the workplace was labeled, tagged or marked with the identity of the hazardous chemicals or with the name and address of the chemical manufacturer, importer, or other responsible party. The employer did not ensure that each container o hazardous chemicals in the workplace was labeled, tagged or marked with the appropriate hazard warnings:    a) On or about July 11, 2013 and at times prior, an uncovered metal container (approx. pint size) said to contain paint thinner was being stored in a cabinet in the shop area. There was no labeling on the container to indicate the identity of the substances inside, no hazard warnings and no name and address for the chemical's manufacturer.  Many paint thinners are composed of petroleum solvent /petroleum distillates (naphtha).           Abatement Note:   Abatement certification is required for this item.
