Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to cuts, amputations, and crushing hazards while working near a moving conveyor:     a)  Denver Post at 5990 North Washington Street, Denver, CO 80216:  On and before July 12, 2013, the employer did not ensure the employees were protected from amputation hazards caused by the use of Conveyor #2 (K&F Printing Systems model APF CR-007, serial number 9703), that was missing two rollers, each with 1 1/2 inch openings, that rotated over fixed conveyor struts.  Condition exposed employees to amputation hazards.      Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard is referenced in ASME B20.1. 2009, Safety Standard for Conveyor and Related Equipment, Section 5.2 Maintenance provides where the lack of maintenance and service would cause a hazardous condition, the user shall establish a maintenance program to ensure that conveyor components are maintained in a condition that does not constitute a hazard to personnel.

29 CFR 1910.178(q)(1):  Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s):  (a)  Denver Post at 5990 North Washington Street, Denver, CO 80216:  On and before July 12, 2013, the employer allowed employees to use electrical forklifts #P8, P16, P17, and P20, with exposed electrical conductors.  Condition exposed employees to electrical shock hazards.
