29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On and before July 10, 2013 the employer did not ensure employees working inside a trench approximately 7 to 9 feet deep, and approximately 36 feet long had a safe means of egress from the trench.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift:    On or before July 10, 2013 the employer did not ensure daily inspections of a trench dug to repair an irrigation system water line was conducted by a competent person prior to the start of work, after every rainstorm and as needed throughout a work shift for evidence of possible cave-ins, indications of failure of the protective system and other hazardous conditions.
