OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees      On or about July 12, 2013 employees were exposed to fall, struck by, and crushing  hazards when working on or around the service rig when the employer failed to properly secure guyline cables supporting the rig mast.

29 CFR 1910.27(d)(2): When ladders are used to ascend to heights exceeding 20 feet no landing platforms were  provided for each 30 feet of height or fraction thereof:         On or about July 12, 2013 an employee was seen climbing and working from the mast of the oil well servicing mobile rig with no landing platform, ladder safety device, or other form of fall protection during the climb exposing the the employee to fall hazards.

29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects:    On or about July 12, 2013 employees  at the work site failed to have thier protective hard hats worn during work activities at or near the rig mast  thereby exposing themselves to potential struck by hazards from overhead work.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  in that an employee was not provided a second means of egress from derrick:     For the employee working from the derrick mast on or about July 12, 2013 the employer did not ensure the emergency escape device was securely anchored and made readily available to provide means of escape from the rod basket in the event of an emergency.      The St. Louis Welll Service company was previously cited for a violation of this occupational safety and health standard or its equivalent standard OSH ACT of 1970 Section (5)(a)(1), which was contained in OSHA inspection number 314935313, citation number 1, item number 1 and was affirmed as a final order on November 23, 2011, with respect to a workplace located at West Star 1-18H, Wanette, OK 74878.
