29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On or about 20 June 2013, an employee without utilizing fall protection, accessed the roof of the apartment building to supervise workers removing and replacing composition roof shingles, exposing the employee to a fall hazard of approximately 18 feet in height to the concrete street below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about 20 June 2013, an employee was accessing the roof of the apartment building without the employer providing fall protection training, exposing the employee to a fall hazard of 18 feet onto the concrete floor below.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access. On or about 20 June 2013, an employee utilized a 20 foot, folding, multi-purpose ladder to access the roof without extending the ladder at least 3 feet above the upper landing surface, exposing the employee to a fall hazard of approximately 18 feet to the concrete street below.
