29 CFR 1926.452(w)(6)(i): Employee(s) were allowed to ride on scaffolds when the surface on which the scaffold was moved was not within 3 degrees of level, and/or not free of pits, holes, and obstructions:  a) Jobsite-Route 202 and Garden of Eden Roads, Wilmington, DE - An employee was operating a Genie scissor lift, Model GS-2646 and was allowed to move the lift while extended over unprotected holes, approximately 10" in diameter and 36" deep which caused the scissor lift to tip over. On or about 7/16/13.
