29 CFR 1926.416(a)(1): When the employee was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means, the employer permitted an employee to work in such proximity to an electric power circuit that the employee could contact the electric power circuit in the course of work: This violation most recently occurred on or about July 10, 2013, and at times prior to; where employees, who were demolishing electrical outlets and overhead lighting and in contact with energized circuits, were not protected against electrical hazards.

29 CFR 1926.416(a)(3): Before work had begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work may bring any person, tool, or machine into physical or electrical contact with the electric power circuit. The employer did not post and maintain proper warning signs where such a circuit exists. The employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken: This violation most recently occurred on or about July 10, 2013, and at times prior to; where employees, who were demolishing electrical outlets and overhead lighting, were exposed to energized electrical power circuits. The employees were not advised of the location of such lines, nor the hazards involved and the protective measures to be taken.
