29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:     (a) Worksite:  An employee voluntarily used a 3M N95 filtering facepieces while cutting masonry block with a Stihl masonry saw.  The employer has not provided the basic advisory information in Appendix D of the respiratory protection standard, as determined on July 16, 2013.     Abatement Certification Required for this Item.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv): (Construction Reference: 29 CFR 1926.59):   (a)  Worksite:  Employees used cut masonry block and mix mortar which contains but not limited to, crystalline silica.  The employer has not provided employees training on the hazards associated with the use of the products, as determined on June 16, 2013.   Abatement Certification Required for this Item.
