29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about July 17, 2013, Chip's Roofing & Exteriors, LLC, as controlling employer, did not ensure that employees of Munsia Roofing, subcontractor, were protected from fall hazards of 9 feet and greater while removing roofing material and gutters from a residential building without fall protection.      Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").    The Chip's Roofing & Exteriors, LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313416380, citation number 1, item number 1, and was affirmed as a final order on February 24, 2010, with respect to a workplace located at 1017 Longs Peak Ave. in Longmont, Colorado.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  (a)  On or about July 17, 2013, Chip's Roofing & Exteriors, LLC, as controlling employer, did not ensure that employees of Munsia Roofing, subcontractor, were safe to use a scaffold on the south side of the building that was being reroofed, in that the scaffold was not inspected by a competent person prior to the employees using it.  Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
