29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service:  a) Jobsite: On July 17, 2013 and at times prior thereto, a fall arrest harness that had been used by the employees on previous occasions, had visible damage which included rips in the harness strap fabric.  Another area of the same strap was torn completely in two with a staple holding the two pieces together.    Note) Abatement certification IS required for this item.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:   a) Front side of home: On July 17, 2013 and at times prior thereto employees used an extension ladder that was positioned at the front rake edge, that did not extend 3 feet beyond the roofs edge.
