29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: On or about July 16, 2013, the employer did not ensure construction debris was cleared from work areas. Employees performing cornice work activities were exposed to slip and trip hazards while walking about the site.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about July 16, 2013, the employer did not ensure electrical flexible cords were equipped with grounding pins while employees performing decking activities. The employees utilized a flexible cord to energize an electric air compressor and were exposed to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. On or about July 16, 2013, employees performing decking activities were working on the roof without fall protection. The employees were exposed to a fall hazards at heights of over 6 ft. to the ground below.
