OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards from impropely used suction hoses.    a) K & D Industries of Ohio, Inc. - Mansfield, Ohio: On or about May 20, 2013, at the worksite located at 132 W. Main St. in Shelby, Ohio: The employer did not assure vacuum hoses were securely attached prior to employees performing vacuum operations.  To vacuum dry waste material, the employer used duct tape to attach a 4-inch diameter hose to a 2-inch diameter hose.  Employees performing the vacuuming operations were exposed to struck-by hazards in that the suction hoses were not properly secured to retain connection.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
