29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:    On or about July 19, 2013, three employees on the Northeast side of the roof were working without any fall protection, and were exposed to a fall of approximately 30 feet in height to the unpaved ground below, while installing metal roofing panels on a steep roof with a pitch of 5:12.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee being exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about July 19, 2013, and at times prior thereto employees on the Northeast side of the roof were exposed to a fall of approximately 30 feet in height to the unpaved ground below, while installing metal roof panels on a steep roof with a pitch of 5:12.
