29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    On or about July 16, 2013, at the worksite located at 9120 Rogers Avenue in Fort Smith, Arkansas, north front of building being constructed, the man basket attached to Skytrak 19596 that was used to elevate employees to designated work areas on roofs was not approved for use by the manufacturer.  This condition exposed employees to tip-over and falls from elevation.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:    On or about July 16, 2013, at the worksite located at 9120 Rogers Avenue in Fort Smith, Arkansas, north front of building being constructed, standard guardrailing was not used across the approximately 9'4" opening on the front of the man basket that was used to elevate employees to designated work areas on the roof to perform truss installation tasks.  Employees were exposed to falling from the man basket to ground surface below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about July 16, 2013, at the worksite located at 9120 Rogers Avenue in Fort Smith, Arkansas,    (a)  North front of building being constructed, employees working on a sloped gable overhang setting wall trusses were not protected from falling to lower level.  Employees were exposed to an approximately 13 feet fall from elevation.    (b)  South back of building being constructed, employees working at an elevation of approximately 13 feet setting trusses were not protected from falling to lower level.  Employees were exposed to falls from elevation.
