29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)   a) Front Right Side of Structure - Employee working from scaffolding equipped with only one plank as a platform was priming and installing an air barrier on or about 7/18/2013.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(g)(1)(vii): For all scaffolds not specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   a) Front Right Side of Structure- Employee working from top level of scaffolding installing an air barrier to the structure not protected from falling 35' to the concrete below, on or about 7/18/2013.   Abatement certification required within 10 days after abatement date. The certification shall include a stateementment that abatement is complete, date and method of abatement, and method of abatement, and states employees and their representives were informed of this abatement.
