29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:   (a) Gas 008: On or about July 3, 2013 and at times prior, Houston Inspection Field Services, LLC, as controlling employer, did not ensure that employees of Sunland Construction, Inc., and Midwest Mole, Inc., were protected from hazards by ensuring frequent inspections were conducted following unexpected, significant changes to the scheduled work activities that affected the jobsite, material, and equipment required to complete the pipe pulling task.    Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
