29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed:   (a)  Employee Break Room and Restroom:  The employer has not kept the employee break room and restroom in a sanitary condition.  The break room was full of trash not located in a trash receptacle and the toilet was covered in dirt and the hand washing sink was full of trash, as observed on July 23, 2013.    Abatement Certification Required for this Item.

29 CFR 1910.305(j)(1)(i): Fixtures, lampholders, lamps, rosettes, and receptacles located had live parts normally exposed to employee contact:   (a)  Back Wall of Plant:  An 120 volt receptacle did not have a cover to protect employees from energized parts of the receptacle.  The receptacle had an extension cord plugged into it that provided power to a battery charger, as observed on July 23, 2013.   Abatement Certification Required for this Item.

29 CFR 1910.179(j)(3): A complete periodic inspection of crane(s) had not been conducted in the past 12 months:   (a)  Plant:  Employees operate a 6 ton Morris Wheeler & Co. Inc. overhung crane to move concrete forms and concrete hopper.  The crane has not had a complete periodic inspection for the past three years, as determined on July 23, 3013.    Abatement Certification Required for this Item.
