29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:        a) Employees were exposed to the hazard of falling approximately 1 story while disposing of roofing materials and debris at the roof edge where fall protection had not been provided.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems:     a) Employees were exposed to the hazard of falling approximately 1 story while hoisting materials and equipment at the edge of the roof deck.   Sanz Roofing Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(3) which was contained in OSHA inspection number 281207, citation 2, item 1a, and was affirmed as a final order on May 2, 2012, with respect to a worksite located at 1736 N. Mohawk Street, Chicago, Illinois.   Sanz Roofing Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(3) which was contained in OSHA inspection number 315508614, citation 1, item 2, and was affirmed as a final order on June 13, 2011, with respect to a worksite located at 400 E. Golf Road, Arlington Heights, Illinois.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
