29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:     A)    Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer failed to ensure that employees, who were trimming candle wicks with scissors, were provided proper personal protective equipment such as, but not limited to, cut resistant gloves. Employees were exposed to laceration hazards.    No additional abatement information is needed for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A)    Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer did not ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating shaft of the conveyor. Employees were exposed to laceration hazards.      B)     Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, moving hood of the re-melter. Employees were exposed to struck-by and laceration hazards.    No additional abatement information is needed for this item.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  A)    Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer failed to ensure that electrical power strips were used as intended. The employer plugged in industrial pedestal fans in electrical power strips that were not listed for industrial use, exposing employees to electrical hazards of 110 volts and to fire hazards.    B)     Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer did not ensure that knockout style metal receptacle boxes were used as intended.  The boxes were not listed to hang down from the ceiling pendent style, exposing employees to electrical shock hazards of 110 volts if knockouts were to be pushed into a box.  The boxes could also have become energized if the conductors were to pull free from being improperly suspended from the ceiling.     Swan Creek Candle Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.303(b)(2)), which was contained in OSHA Inspection # 311610554, Citation number 1, Item number 1, and was affirmed as a final order on 09/14/2009, with respect to a workplace located at 395 West Airport Highway, Swanton, OH 43558.    No additional abatement information is needed for this item.

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.    A)    Ambrosia, Inc. doing business as Swan Creek Candle Co., located at 395 West Airport Highway, Swanton, OH 43558: On or about July 23, 2013, the employer did not ensure that flexible cords were protected from accidental damage that could be caused by employees stepping on cords because cords were placed on the floor in the walkways. Employees were exposed to electrical hazards of 110 volts.      Swan Creek Candle Co. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1910.305(a)(2)(x)), which was contained in OSHA Inspection # 311610554, Citation number 1, Item number 2, and was affirmed as a final order on 09/14/2009, with respect to a workplace located at 395 West Airport Highway, Swanton, OH 43558.    No additional abatement information is needed for this item.
