29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:   a)  Koch Container, on or about 7/22/13:  The Employer did not provide annual training to employees on the use and application of portable fire extinguishers.    *ABATEMENT NOTE:  By this date the employer must either correct the alleged violation or implement a Fire Safety Policy; as outlined in 29 CFR 1910.38(a) and .39(a) which includes the evacuation requirements of 29 CFR 1910.157(b).

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  Maintenance, on or about 7/22/13:  Clausing lathe was not provided with a chuck guard.  b)  Maintenance, on or about 7/22/13:  Jet drill press was not provided with a guard to cover the serrated chuck or cutting tool.  c)  Manufacturing Area, on or about 7/22/13:   Hycorr Die Cutter, scrap conveyor positioned at floor level was not guarded to prevent employees from contact with the moving belt .    d)  Manufacturing Area, on or about 7/22/13:  Ward Die Cutter, scrap conveyor positioned at floor level was not provided with a guard to prevent employees from contact with the moving belt.
