29 CFR 1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.     a)     On or about July 23, 2013, at the worksite, the employer did not perform a workplace hazard assessment through written certification that would identify hazards and types of personal protective equipment such as, but not limited to safety glasses, leg chaps, hard hats and hearing protection, needed for employees performing tree cutting work with a saw zaw.

29 CFR 1910.266(d)(1)(vii): The employer shall provide, at no cost to the employee, and assure that each employee wears the following:  29 CFR 1910.266(d)(1)(vii)(A): Eye protection meeting the requirements of subpart I of Part 1910 where there is potential for eye injury due to falling or flying objects.     a)     On or about July 23, 2013, at the worksite, the employer did not ensure that employees were provided and used eye protection while using a saw zaw to cut down a tree, thereby exposing employees to a potential struck-by hazard.

The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective.   a)     On or about July 23, 2013, at the worksite, the employer did not ensure that the extension cord that was used to energize the reciprocating, was equipped with a ground pin, thereby exposing employees to a potential shock hazard.
